This plan gives you peace of mind knowing that when you are dying your loved ones and carers know what you want so you can have the dying experience you want. Your MyDyingPlan (MDP) is your final opportunity to have your say. It ensures that medical decisions are made according to your choices and that your preferences for your last moments can be respected.

Your MyDyingPlan is a document, sometimes called a Living Will, that captures your decisions for medical treatment, care, and comfort when you are dying and cannot speak for yourself. It provides guidance for your loved ones, caregivers, and doctors for your final moments.
Once you’ve put in the work to define your wishes; don’t let it go to waste by keeping it to yourself. If the right people don’t know what matters to you, they cannot advocate on your behalf when it counts. Sharing your plans is just as important as making them.

Medical directives (e.g. resuscitation, ventilation, medication )

A Last Will and Testament outlines what happens to your assets after you die. Your MyDyingPlan focuses on what happens to you while you are dying, specifically when you unable to speak for yourself. Both documents are important and serve different important purposes.

MyDyingPlan is a form you fill in. There are a number of questions to answer and spaces where you can add additional information as required.
There is a space for you to sign and date the document, as well as for a witness or your lawyer’s signature.

It ensures everything is organised and stored in one convenient location for easy access when needed. It makes it much easier for someone to handle your estate if you’re unable to or to close it down after you die.
Creating your MyOne Place is only half the job. If you don’t share it with the right people, they can’t step in and manage your estate when it matters most. Your voice remains unheard if you don’t communicate this information with your trusted people.

A non-medical professional who provides emotional spiritual and practical support to individuals and their loved ones during the dying process. Dying is a deeply personal journey and as such requires collaboration to create a tailored experience to meet your needs.

MyDyingPlan and MyOnePlace have been created so that they are easy to complete. That doesn’t mean this is not difficult work to do. We understand that death is a heavy topic and thinking about your own death can bring up a lot of feelings. It can also just be hard to know where to start and some help could make it ‘easier’. So, we offer one on one support to guide you through completing these documents. By the end, you'll either have the documents fully completed, or leave with a clear path forward.
